What is Dermal Filler?

Dermal filler used at Taunton Aesthetics is a hyaluronic acid (HA) which is a naturally occurring sugar in our bodies. Just 1g of it can hold up to 6 litres of water. It is essential for moisture and volume in our skin. The dermal fillers we inject are used to replace lost volume, treat lines not caused by muscle activity and to treat asymmetries.

How does it work?

The gel-like substance is injected into the skin which has initial volume to it and creates lift but its longevity works by attracting water to the molecules creating a more voluptuous and fresh appearance.

Is it safe?

Dermal fillers are very safe when used correctly and by experienced practitioners. It is advised all treatments of this kind be done by someone with a professional or governing body over seeing them to ensure they are accountable for their actions. As nurses and prescribing nurses, we can ensure safe practice is adhered to and your safety is of our most concern.

If problems do arise, we can quickly dissolve the hyaluronic acid filler (HA), this is a rare occurrence.

Cheek Contouring

As we age the apples of our cheeks begin to flatten as the fat pads beneath them start to separate and reabsorb. Having some filler to the apples of the cheeks and where the separation can be seen provides you with a much more rejuvenated appearance, treatment can also be done along the cheek bone where the fat pad has reabsorbed. Having cheek filler can also help improve the lower face contouring as it provides lift and structure to the areas below it. In particular, the nasolabial fold and marionette lines. The shadowing is reduced by the lift, casting the eye away from this area which then only a small amount of product may be used or may not have to be treated at all.

Jaw Line Contouring

At Taunton Aesthetics we offer jaw line contouring for men and women of all ages. Jaw enhancement can be sought because of an aging appearance or just because you want a more defined chiselled looking jaw.

The pre jowl sulcus is the indentations along the length of the jaw line below the mouth interrupting the smoothness of the jawline creating a heavy and aging appearance. Adding filler to the back of the jaw creates a beautiful sharp or rounded angle and along the jaw line to even out the bumpiness, resulting in a much more youthful or strong enhancement.

Lip Augmentation

Lip enhancement can be sought after for women and men of any age over 18. As we age lips loose volume and structure, sometimes the corners of the lips turn downward, or the peak of the cupid’s bow becomes flat and no longer defined. Sometimes we are born less graced than others and we never had the volume or defined lip edges that some are blessed with. By injecting HA fillers, we boost the HA content to create a fuller more youthful appearance.

Marionette Lines

Aptly named due to the old-fashioned puppets mouth. Lines that run vertically down from the corners of your mouth. Again, due to aging, the lack of skin support, fat reabsorption and lack of mid face volume, the skin pulls down. Treating this area with dermal filler reduces the shadowing and minimizes the folds creating a much-lifted appearance to this area.

Oral commissures

Technically a part of the marionette line. It is the area where the corners of your mouth meet your cheek. Some people get a hollowing or a pulling down of the corner of the mouth resulting in a sad look even though you could be on top of the world. We are happy to treat this are in combination of other treatments resulting in a lovely lift or on its own. The marionette line continues down from this corner of the mouth.

Nose to Mouth Lines (nasolabial)

One of the key areas that trouble clients is the nasolabial lines. This is mainly due to the cheeks losing support and volume, creating a sag or casting a shadow to this area, it can also be due to the fat pads separating beneath the surface of the skin. This area can create a droopy appearance and make us not feel as young as we once was. The ideal treatment is to readdress the volume to the cheeks creating more lift to the nasolabial fold and taking the eye away from this area but we understand that is not always possible due to your budget. However dermal filler beneath these creases using different techniques can create a beautiful fresh looking result for many people.

Perioral lines

As we age, we can start to notice that our lipstick starts to bleed from your lip line or that horizontal lines start to develop around your mouth, these were once known as smokers’ lines. These lines are caused by regular muscle activity of the lips and natural volume loss around that area. They can cause a real dip in confidence for many men and women as they can make us look much older that we really are. By using HA fillers to around the vermillion boarder (natural lip line) we can create a more defined lip edge and with different approaches and techniques we can really soften the lines around the mouth to give you back your confidence.

Tear Trough

Dark shadows under the eyes can be a bugbear for many patients, an endless amount of concealer trying desperately to hide that tired look to no avail!

Tear trough deficiency or dark circles caused by hollows can at times be a hereditary thing, look at your mum and dad, if they have them, the likelihood is you may have them.

Some people develop dark shadows are they age, and certainly different environmental factors affect this area too such as sun damage, free radical damage, and hormonal changes resulting in pigmentation and skin laxity.

Under the eye, we have fat pads, and like all fat pads they shrink, they descend and, in this area, can also protrude or prolapse with age.

At Taunton Aesthetics we can treat a hollow if it is lacking volume with dermal filler, carefully placed to lift the skin and to reduce the shadow caused by the deficit. For the very best advice in this area, sometimes it will be necessary or advisable to treat the cheeks prior to the tear trough as this treatment may be able to improve the undereye area on its own and will provide a better harmony to the ratios to your face.

Not all dark circles are caused by hollows, certain ethnicities have dark under-eye circles, generally those who have darker skin types. These circles aren’t necessarily a hollow or volume deficiency, it can often be pigmentation such as melasma. These circles won’t be improved very much with the use of dermal filler on its own. Skin lightening treatments will need to be incorporated into skincare if this really bothers you.

What’s the difference between toxin injections and dermal fillers?

Botulinum injections work on the deactivation of muscle activity. No movement can result in smoother-looking skin as the skin is no longer being folded.

Dermal fillers are a medical device containing hyaluronic acid which is a natural substance already in our skin. It is injected into the skin to lift and plump it to create volume where it has been lost.

What are differences between all the Hyaluronic acid brands?

Essentially, they are all the same, they contain HA, but there are some variants. Some contain lidocaine which is a local anaesthetic ingredient. The brands vary due to their molecule sizes, the substances they are made from and the areas they are made to treat and their longevity.

If you have a preference, we will discuss whether it is ok for you to have. Depending on previous treatments you may need to have the same product you have been treated with previously.

We only buy from reputable pharmacies to ensure the product we inject is safe and reliable to be what it says it is.

When will I see the results of my dermal filler injections?

Instantly! Unlike toxin injections that take a few weeks to see the full results you will see what we have created there and then.

Does it hurt?

Some areas are more uncomfortable than others. We are happy to provide local numbing cream to the areas being treated to help the initial pricks being felt, people are pleasantly surprised by how comfortable the procedure is as most of the fillers we use have a local numbing agent in them, unless you are allergic to lidocaine or on certain medications. Some areas treated don’t require local anaesthetic but again this is a personal choice, we are happy to numb you up!

What are the side effects of HA filler?

The most common and desired effect is volumized areas of skin.

Reactions around the injection site due to the puncture of the skin rather than the product are as such: redness, tenderness, itching, swelling or bruising are common.
Minor bumps which can be improved with massage.

If you are prone to cold sores you could expect to get a flare-up of the virus and we advise having started your treatment a few days prior to injections.

Very rare- Injecting product into the small vessels causing reduced blood supply, scab formation or scarring.

Patients who are particularly nervous could experience generalised reactions such as fainting, circulatory problems, nausea or noises in the ears. Relax we will assess you and make sure you leave the clinic well and at ease. Everyone can be afraid, but we won’t push you into anything you don’t want to do.

What if I don’t like the effect?

We only use HA fillers as these can be dissolved. We only really dissolve if there is a medical emergency warranting the dissolvement of the product. We would dissolve also if the product we injected wasn’t aesthetically pleasing, but if there is nothing wrong with the look we advise that you let nature take its course and let your body metabolise it naturally as there are risks involved with using the agent Hyaluronidase. We will have a full consultation regarding this treatment plan if this is the way you would like to proceed.

How long does it take?

Depending on the brand of product used and where we put it varies, but in general you will see the results lasting between 6-9 months. Some of the thicker premium fillers can last 18-22 months.

Will I need a consultation?

Yes, you will need to come for a pre-treatment consultation to take a full history etc and to order your product in.  You will need to make a second appointment for your treatment.  This time give you enough headspace to make an informed decision on whether this is the treatment for you. 

How long does it last?

Depending on the product used and where we put it varies, but in general you will see the results lasting between 6-9 months. Some of the thicker fillers can last 12-18 months.

My filler didn’t last very long!

Everyone is different, but the average person can still see/ feel their product for 6-9 months depending on which one was used.

Depending on your metabolism or degree of physical activity you do, you may find your filler treatment burns off quicker than someone with a more sedentary lifestyle.

What shouldn’t I do after my treatment?

Most people can resume normal activity straight away as there is very little down time, but we suggest you leave the area treated alone for a few hours to reduce the risk of infection.

We suggest for 24 hours no vigorous exercise, drinking alcohol or exposed sun exposure succeeding your injections.

Who can’t have dermal filler injections?

A pregnant or breast-feeding woman cannot have treatment.

People who have severe allergies or anaphylaxis should not have dermal fillers
Patients on a course of antibiotics.

Patients on Roaccutane for acne.

Can I fly after having dermal filler injections?

Yes, you can but we advise you wait 7 days before going on any long haul flights, as the increased cabin pressure for such long periods of time can disturb the setting of the filler and can increase bruising.

How long do I have to wait before my next filler injections?

Depending on where you have been treated can mean when you can have a top up. On average though we advise having top ups between every 6-12 months in the lips. Cheeks can be reinjected after 2 weeks until the right volume has been restored. If a beautiful result has already been achieved, we wouldn’t generally give more until the product starts to wear off. Fillers last between 6-18 months depending on brand and placement. Over correcting or over filling can result in the filler becoming lumpy, migrated or a general poor aesthetic result. Sometimes less is definitely more!

Can I have other treatments after/before my injections like peels, needling or semi-permanent makeup?

Yes, you can have a peel immediately before a filler or toxin injection as long as it’s done by the same practioner. If you choose to have micro-needling or derma abrasion its best to wait a few weeks before getting your treatment.

If getting treatment of filler or toxin by another clinician wait until your skin is fully healed before having your treatment.

It is better to have had micro-needling/derma abrasion two weeks before toxin or filler treatment.

Semi-permanent makeup and treatments ideally should be done first. This is because having the tattoo first can give you a lovely shaped eyebrow at natural rest and the toxin injections will just bolster the result.

Having an eyebrow lift with either toxin or filler before having the tattoo gives the tattooist or beauty technician a newly symmetrical face after two weeks, but to remain symmetrical you will need to keep up with toxin /filler injections – your finances will have to be considered. This is because as your face returns to a normal resting position i.e. the toxin has worn off your eyebrows could look a bit uneven/symmetrical.

If you do choose to have semi-permanent, make up such as lip line or eyebrows you should wait 2 weeks before having your filler to ensure you are fully healed.
